Become a Patient: What to bring on your First Visit
To become a patient at SCCHC, you will be asked to bring the following items to your intake visit:
- A denial letter from Medicaid that you are not covered as a Medicaid recipient. This can be obtained at no cost on a walk-in basis at the Health Department.
- Proof of residence in St. Clair County. If you driver’s license is current and lists your address in St. Clair County, this will be acceptable. If not, you can bring a copy of a bill or anything mailed to your current address.
- Proof of your income which may include a copy of your bank statement, a copy of your last year’s tax return, or recent paystubs.
- The intake process costs $20 which will be collected at the time of your intake visit.
