Become a Patient: What to bring on your First Visit

To become a patient at SCCHC, you will be asked to bring the following items to your intake visit: 

  1. A denial letter from Medicaid that you are not covered as a Medicaid recipient. This can be obtained at no cost on a walk-in basis at the Health Department. 
  2. Proof of residence in St. Clair County. If you driver’s license is current and lists your address in St. Clair County, this will be acceptable. If not, you can bring a copy of a bill or anything mailed to your current address. 
  3. Proof of your income which may include a copy of your bank statement, a copy of your last year’s tax return, or recent paystubs. 
  4. The intake process costs $20 which will be collected at the time of your intake visit.
