St. Clair Community Health Clinic Receives $15,000 CVS Health Community Grant

St. Clair Community Health Clinic has received a $15,000 grant from CVS Health, as part of the company’s commitment to building healthier communities. The support from CVS Health will help the St. Clair Community Health Clinic expand their telehealth program and increase the accessibility, capacity, and effectiveness of the services offered.

The St. Clair Community Health Clinic (SCCHC) is a non-profit 501c3 and the only entity providing primary/specialty healthcare series for uninsured residents in St. Clair County, Alabama (ages 19-64 and at or below 125% of federal poverty guidelines).  SCCHC is currently operated 100% by volunteer medical providers.  Due to the impact of the COVID-19 pandemic, the SCCHC has transitioned to a primarily telehealth model.  The CVS Health Community Grant funds will be instrumental in expanding the telehealth program and increasing the accessibility, capacity, and effectiveness of the services offered.  Specifically, the grant funds will provide a much-needed upgrade in technological resources to support the clinic and the telehealth model.  The grant funds will also be used to hire a part-time program administrative assistant to give SCCHC consistency when dealing with volunteers, provides, patients, and the overall administration of the program.

To date, the St. Clair Community Health Clinic has served more than 750 individual patients and over 2,500 total patient visits completed. The goal of the clinic is to treat all patients with dignity and respect, providing care for the body, mind, and spirit and thus elevating quality of life. The primary outcome measure for the clinic is to decrease emergency room use in active clinic patients by providing routine care and medications, managing chronic illnesses, and promoting a culture of wellness. The long-term goal of St. Clair Community Health Clinic is to improve the overall health of the county through education and support of patients, empowering them to take ownership of their health and obtain a level of wellness previously thought to be unreachable.
