To become a patient at SCCHC, you will be asked to bring the following items to your intake visit:

Health and wellness are promoted through care of body, mind, and spirit to achieve the wholeness that God created us to live in while providing high quality primary and specialty care medical services to a diverse population.

Purpose

St. Clair Community Health Clinic It is a medical mission that sees St. Clair County adults who are at or below 200% of the Federal Poverty Guidelines and do not have access to Medicare, Medicaid, or VA Benefits.

Medicaid Denial Letter

A denial letter from Medicaid that you are not covered as a Medicaid recipient. This can be obtained at no cost on a walk-in basis at the Health Department.

Proof of residence

Proof of residence in St. Clair County. If you driver’s license is current and lists your address in St. Clair County, this will be acceptable. If not, you can bring a copy of a bill or anything mailed to your current address.

Proof of Income

Proof of your income which may include a copy of your bank statement, a copy of your last year’s tax return, or recent paystubs.

$20

The intake process costs $20 which will be collected at the time of your intake visit.
